Dr. Kerry Gelb
Dr. Kerry Gelb received his Bachelor's of Science Degree from SUNY College at Oneonta in 1980. He graduated with honors in 1984 from the Illinois College of Optometry, and completed a one year residency in the Diagnosis and Management of Ocular Disease. Dr Gelb has been practicing in Woodbridge, N.J. for the past 22 years. He is a member of the NJSOP, the New Jersey Society of Optometric Physicians, and the president of the Association of LensCrafters Leaseholding Doctors.
Dr. Gelb has been interviewed on both radio and television on topics relating to ocular health. He frequently lectures on the diagnosis and treatment of Macular Degeneration, Glaucoma, and Diabetes. Dr Gelb has extensive knowledge and experience in the evaluation and fitting of specialty or "hard to fit" contact lenses. He is certified in the evaluation and management of Corneal Refractive Therapy contact lenses. He is committed to providing the highest quality of care to his patients. He resides in Union County with his wife.
Dr. Jason Yee

Dr. Jason Yee has been practicing with Contact Lens & Vision Consultants since 2001. He is a graduate of St. John’s University where he earned a B.S. in Biology. He received his Doctor of Optometry degree from the State University of New York College of Optometry. Dr. Yee has also completed a residency in Family Practice Optometry and ocular disease at the East New York Diagnostic & Treatment Center. Dr. Yee specializes in contact lens fittings and is also trained and certified in the treatment and management of ocular disease. Dr. Yee is a Brooklyn native and is happy to provide comprehensive eye care to his hometown community. |
Dr. Jeffrey Case

Dr. Jeffrey Case has been practicing therapeutic optometry at Contact Lens & Vision Associates located at Brunswick Square Mall and Freehold Raceway Mall for 20 years. He received his bachelor's degree from Alfred University in 1984. He received a Doctorate in Optometry from New England College of Optometry in 1988. He completed his residency in ocular disease at Omni Eye Services in Iselin, NJ. Currently he is a member of the American Optometric Association & New Jersey Optometric Association. In addition, Dr. Case is licensed to practice optometry in the State of Pennsylvania and New York.
Dr. Case currently resides in Morris County with his wife, Susan, also an optometric physician, and three daughters. |

Dr. Kruti Patel

Dr. Kruti Patel provides comprehensive optometric care. She received her bachelor's degree in Biological Sciences from Rutgers University. She received her Optometry Degree from Pennsylvania College of Optometry in 2005. Externship experience includes a rotation at the Newington Veteran's Hospital in Connecticut and at Eye Centers of South Florida in Miami Beach. Professional experience includes ocular surgery co-management, specialty contact lens fitting, diagnosis and management of ocular disease, ocular trauma and primary care optometry. She is also certified with Paragon Vision Sciences for a special contact lens-based procedure called Corneal Refractive Therapy. Dr. Patel is a member of the American Optometric Association and the New Jersey Optometric Association. Dr. Patel lives in Middlesex County, NJ enjoys outdoor activities, traveling and reading. She speaks English, Hindi and Gujurati fluently. |

Dr. Avanti Rajan
Dr. Avanti Rajan received her Bachelor of Science from the University of Michigan in 1998. She continued her education at the Michigan College of Optometry, where she graduated in 2002 with honors. Dr. Rajan specializes in primary care optometry, contact lens, geriatrics, and disease management. She is also certified in Paragon Corneal Refractive Therapy. Dr. Rajan is a member of the New Jersey Society of Optometric Physicians and the American Optometric Association. She is fluent in Marathi. In her spare time she enjoys traveling, reading, and playing tennis. She currently resides in Iselin with her husband, Dr. Samir Rajan.
Dr. Gary Strickland
Dr. Strickland is the primary doctor at our Freehold office. Having graduated from the Pennsylvania College of Optometry in 2009, he has done rotations in the areas of contact lens and in the treatment of ocular disease. Having worked six months in the VA Hospital of Delaware, he is trained on the diagnosis and treatment of glaucoma, macular degeneration, diabetic changes of the eye, cataracts, as well as other age related changes which can affect vision. He has also worked with Joel Silbert, a well known contact lens specialist in New Jersey and the educator at Salus University and is trained in specialty contact lens fittings, including the use of bifocal contact lenses and the treatment of keratoconus. He is available for the treatment of ocular emergencies and has great relations with local surgeons in the area should immediate care be needed.
Parents find that Dr. Strickland is great with children and he makes the eye exam fun, making video games, school, and sports the highlight of the exam. He prides himself on explaining to his patients what he looks at in the eyes, what changes he expects to see, and how he can help provide the most comfortable and clear vision to those who visit him.

Dr. Pham
Dr. Pham is a member of our professional staff. He earned a Bachelor of Science in Visual Science and his Doctor of Optometry degrees from the Pennsylvania College of Optometry at Salus University in Philadelphia. He is Nationally Board Certified to diagnose and treat ocular diseases. Dr. Pham enjoys prescribing contact lenses and welcomes challenging cases requiring complex lens materials and designs. He is certified in Paragon Corneal Refractive Therapy. He also cares for patients who have had refractive surgery (LASIK, PRK, etc.). He is an active member of the American Optometric Association. In addition, Dr. Pham is licensed to practice optometry in the State of Pennsylvania.
Dr. Pham is fluent in Vietnamese. He enjoys traveling and outdoor activities.

Dr. Manoj Khanna

Dr. Khanna came to US as an immigrant from India at age eleven. This is when he had his first eye exam ever. From that day forward he wanted to be an Optometrist. He received his Bachelor of Science from York College of PA. He attended graduate school at Johns Hopkins University and earned his Doctoral degree from PA College of Optometry, regarded as the premier primary eye care institute in the country. He is a member of the NJ Society of Optometric Physicians, American Optometric Association and PA Optometric Association. His current patient base is young as 3 and as ripe as 90. Dr. Khanna is fluent in English, Hindi, Punjabi, and Urdu. He is also able to perform eye exams in Spanish. |

Dr Choe received her undergraduate degree from Smith College in Massachusetts and then completed her Doctoral degree from Pennsylvania College

Dr. Yanique Forrest

Dr. Yanique Forrest earned her Bachelor’s degree in Biology from Stony Brook University in Stony Brook, NY in 2006. After participating in SUNY Optometry’s CSTEP program in 2005, she developed an interest in and decided to pursue further education in the field of optometry. She earned her Doctoral degree in 2011 from SUNY College of Optometry. Her training includes clinical rotations in Primary Care Optometry, Vision Therapy, Low Vision, Pediatrics, and Ocular Disease. Dr. Forrest's externship experiences allowed for further training in Comprehensive eye care and Ocular Disease at the Northport VA Hospital and at the East New York Diagnostic & Treatment Center.
Prior to, and while attending optometry school, Dr. Forrest also worked with Contact Lens and Vision as an Optometric Tech. She is proud to now be able to provide quality eye care alongside her colleagues with whom she has worked with since 2006.
Dr. Forrest is of Jamaican heritage and currently resides in Brooklyn. She enjoys traveling and spending time with her family.

Dr. Phillips provides professional care for all patients equally and continuously broadens her knowledge and skills to serve them with the most updated and proper care. She earned her bachelor’s degree from Rutgers University then received her Doctor of Optometry degree from Pennsylvania College of Optometry at Salus University. Dr. Phillips provides full-scope management of ocular infection, inflammation, and ocular trauma.She manages patients' primary eye care health, including diagnosis and treatment of glaucoma, anterior and posterior segment disease, pre-and post-operative care, and emergency eye care.She also enjoys fitting, prescribing and counseling patients on contact lenses. She is patient and focuses on patient education on all procedures and results. On her leisure time she enjoys traveling, swimming and spending time with her family. |
